Authority to release medical information

The case manager may need to contact health providers for a medical report or to discuss the nature, extent or probable duration of the injury.

The injured worker’s authority is required before this can happen. The injured worker can sign the medical authority section of the claim form, and/or the case manager can send them a medical authority.
